**Job Role**: Prison Resettlement Worker
**Salary**:£24,500
**Hours**:Full-time 37.5 hours per week
**Contract type**:9 Month FTC (possibility for extension)
**Location**:Taunton and Yeovil.
**The Role**:
To deliver a person-centred, wrap-around outreach support service, actively case-working complex needs and vulnerably housed individuals to prevent rough sleeping amongst prison leavers. Working closely with the existing Julian House supported housing services in the Sedgemoor area, to manage referrals into the service and the completion of assessments, where required.
**Responsibilities include**:
- To recognize indications of exploitation, offending, substance misuse, mental health needs, domestic abuse, personality disorder behavioural issues and make appropriate referrals to relevant agencies.
- Empowering clients to take responsibility and to develop their skills, confidence and resilience.
- To promote and facilitate effective multi-agency and joint working with South Somerset LA, Police and Probation services to reduce exploitation and involvement in serious organized crime.
- Keeping full and accurate records, producing statistical and analytical information for monitoring purposes and carry out all necessary administration in relation to case work carried out.
**Qualifications / Requirements**:
- Knowledge of the criminal justice system, prison leaver resettlement and or Housing Options process.
- Work flexibly towards creative solutions in a person-centred and trauma informed approach with individuals
- Have full driving license and access to own car.
- Working with complex need people who are homeless or at risk of homelessness.
