**The role**:
**Community Care Worker - Over 65 Review Team - Fixed term until 30/08/2024**

**Position Overview**:
As a Community Care Worker on the Over 65 Review Team, you will play a vital role in reviewing the support individuals are receiving. Your primary responsibility will be to review care plans, making necessary adjustments to ensure they remain effective and meet changing needs.

**Responsibilities**:

- Assist the social work team in conducting Care Act assessments and reviews (this may be by telephone or in person) with adults over 65, taking into account their physical, emotional, and social needs.
- Collaborate with the social work team to develop and implement person centred and strengths based care plans based on the individuals' needs and preferences.
- Communicate effectively with healthcare professionals, care providers, and external agencies to coordinate appropriate support for individuals.
- Offer information, advice and guidance to older adults and their families, ensuring their rights and choices are respected.
- Maintain accurate and up-to-date documentation and records in accordance with established policies and procedures.
- Stay informed about changes in legislation, policies, and best practices.
