An exciting opportunity has arisen for a Care Co-ordinator to work for the Amber Valley and Erewash Rapid Response Team. You will be joining a busy, friendly and supportive team.

The role of this team is to support patients at home with complex health needs. We promote a model of self care
- increasing independence to prevent avoidable admissions, support early discharges from hospital and allow people to remain at home for as long as possible where able.

The role of the Care Co-ordinator is pivotal to our service, supporting the clinicians and wider multidisciplinary team in providing quality patient centred care. You will be working alongside the Community Response team (Carer support in the community), Rapid Response Nursing and Therapy Teams.

You will be working closely with our team, dealing with daily admin requests and you will play a key role in meetings, need to be confident in collecting accurate data and undertaking audits as well as working alongside our wider MDT as we move towards an integrated model with social care. Good prioritisation of tasks is key.

The Care Co-ordinator will be part of the Integrated Community Team who respond to patients needs quickly
- in some cases we have a 2 hour response time and the patients stay within our care for a relatively short period before discharging or referring on.

The Care Co-ordinator is a pivotal role and will be the interface between service users, carers, primary care, secondary care, community care, social care, mental health, out of hours and voluntary organisations.

You will be required to offer various support to the Rapid Response Team, prepping for meetings and case reviews, monitoring and responding to tasks, escalating any patients in delay and maintaining and updating spreadsheets, completing audits and updates.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

**You will**:
Be proactive in supporting the MDT

Liaise with social care, EMAS, Care Co-ordinators, health professionals, local volunteer services and local services.

Follow through with service users and others involved to ensure we have good patient flow through our services.

Manage the meetings process, including taking and distributing minutes of meeting

**You will have**:
Previous, relevant experience of working in a health or social care profession

A proven record of excellent written and verbal communication skills and interpersonal skills

Be creative and tenacious in finding solutions to difficult problems

Good organisational skills
