Advanced Therapy Support Worker

We are seeking 2 x Advanced Therapy Support Workers with a patient-centred approach and a passion for rapid care.
We are developing a new service within Rapid Response providing assistance to people who have fallen but are unable to get up without help. These are patients that historically wait for a long time for assistance as they are not a priority for the ambulance service.
Our team will be provided with the right training and equipment to support these patients back up off the floor, and provide ongoing care as needed in their own homes. Reducing patient waiting times and the pressure on the ambulance service where possible.
Alongside this you will be part of the Rapid Response team. Our integrated, multi-disciplinary team provides timely intervention that is individualised, goal-focussed and support patients in their own homes and local care homes.
The Rapid Response service is provided 8am-6.30 pm 7 days a week.
The service aims are to prevent unnecessary admissions and facilitate timely discharges from acute and community beds, allowing people to remain in their own homes for as long as they are able.
You will be well supported in the role by a skilled, multi-disciplinary team, including Physiotherapists, Occupational Therapists, Nurses, Support Workers and Care Co-ordinators.
As part of the role there will be a need for extensive travel and transportation of equipment around Amber Valley and Erewash.
To assist patients from the floor following a fall, using the appropriate equipment and assessment skills. To perform and assist in the provision of holistic, high quality, evidence-based health care.
To work within an appropriate supervisory and monitoring framework, following assessment proformas, agreed multi-disciplinary treatment plans and by providing and managing ongoing treatment, teaching and instruction to a referred caseload of patients.
The post holder will be expected to implement, modify and evaluate these programmes of treatment without direct supervision.
This will include rapid intervention, triage, initial assessment and reviews of patients, creating specific patient led care plans, meeting goals and liaison with other specialist services within the Trust.
The post holder will organise and prioritise their own day
- to-day work tasks and activities, be able to work independently and adhere to Trust Lone Working policies, as well as delegate tasks to band 3 colleagues and support administrative tasks for the day-to-day running of the services.

To manage a delegated caseload to provide holistic care to patients in their own homes or alternative health care setting, to promote recovery and encourage and maintain independence.
To communicate information to patients and carers regarding their health needs and associated treatment plans.
To report and take appropriate action regarding any change in patient condition or circumstance and to communicate verbal and written information to all relevant integrated care team members.
To assist in the assessment and recording of patient outcome measures.
To support senior therapy staff in the on-going development of services
To maintain and record accurate health records using Systmone/TPP in line with DCHS policies and procedures and Caldicott requirements.
To observe personal duty of care when using or supplying equipment, to ensure compliance with health and safety regulations and to monitor and report to appropriate registered clinician on the effectiveness of equipment and aids to patients’ homes to maximise use of resources.
To ensure “Every Contact Counts” and contribute to public health and health promotion initiatives.
