The primary focus of the role is to support the provision of our Assisted Discharge Service for non-Covid patients who are being discharged from either Eastbourne District General Hospital or the Conquest Hospital in East Sussex. Driving patients home when discharged (using Red Cross vehicles) and making sure they are safely settled at home with everything they need. You will also support them by signposting and referring patients to other sources of support in the community. We want all service users to have a positive experience in the transition from hospital to home.

You will provide reactive, focused and flexible support to adults either following a hospital stay to enable early discharge when clinical needs have been met or to those living in the community to prevent hospital admission. You will assist the Independent Living Service Coordinator by undertaking duties required to meet the service users’ needs in order to re-enable service users to remain in their own home and prevent hospital re-admission.

You will respond to referrals from Clinicians and Community Health Professionals and will be required to have the ability to assess service users’ needs, complete and follow a support plan for the individual. In addition, you will be providing practical and emotional support in the service user’s own home and signposting to other services where appropriate to enable the individual to live independently in their own home. “

**Please note**: You will also need a valid UK driving licence as this role requires travel within the East Sussex area.

**A day in the life of a Service Support Worker will involve**:

- Providing practical and emotional support (telephone calls, welfare checks, shopping etc) and guidance to service user.
- Providing reactive, focused, and flexible support to adults following a hospital stay to enable early discharge.
- Responding to referrals from Clinicians and Community Health Professionals
- Assess service users’ needs, complete and follow a support plan for the individual.
- Supporting service users in the weeks after hospital discharge or to prevent readmission.
- Taking someone to a hospital appointment that they otherwise couldn’t get to or be picking up prescriptions and shopping.

**To be a successful Service Support Worker, you’ll**:

- Have a full UK manual driving licence.
- Have a good knowledge of services provided by the NHS and Social Care.
- Be able to make things great. You know how to improve service quality for the benefit of users.
- Be professional. You can deal with queries in a diplomatic and confidential manner.
- Love being flexible. Working hours out with the norm suits you.
- There is no personal care or administration of medication for this role.
