We have an exciting new opportunity for a Band 5 Nutrition Support Development Dietitian to join our friendly and progressive community team. The successful applicant will progress to a Band 6 role after approximately 12 months, or on completion of the required training and competencies.

The Nutrition Support Service covers clinical caseloads for malnutrition / appropriate prescribing, adult home enteral tube feeding, and complex obesity. The role will require some face to face domiciliary and nursing home visits in the borough of Barnet which makes a car driver preferable. We have a base at Finchley Memorial Hospital where we provide clinics which may be face to face or virtual, and this will allow some home working. The team meets fortnightly to support communication, team building and staff well-being and as this is a developing role you will receive regular clinical supervision.

The successful applicant will mainly support patients in the community at risk of malnutrition by liaising with patients, relatives and carers around a food-based approach and the appropriate prescribing of oral nutritional supplements in line with our CLCH pathway and North Central London (NCL) formulary. There will also be scope for gaining experience in supporting patients on Home Enteral Tube Feeding. You will regularly communicate with GP practices, hospital dietitians, pharmacists and community based nursing, One Care Homes and Frailty MDT’s and contribute to various networks across CLCH and NCL.

You will gain competency in student supervision and be encouraged to take an active role in supporting dietetic assistants and the wider teams activities to build on your own professional and leadership development.

Central London Community Healthcare (CLCH) is one of the largest community healthcare organisations in London and Hertfordshire, providing our services to diverse communities/boroughs in 11 London Boroughs - Barnet, Brent, Ealing, Hammersmith & Fulham, Harrow, Hounslow, Kensington and Chelsea, Merton, Richmond, Wandsworth, Westminster - and Hertfordshire.

The post holder will provide Dietetic assessments and treatment programmes to clients in their own homes and in care homes within the borough of Barnet.

To be responsible for one’s own caseload of clients who are predominantly at risk of malnutrition while working to service standards under the supervision of a Senior Dietitian.

To provide support to the continued implementation of the Nutrition Support Care Pathway for highlighting and treating malnutrition in the community as well as the appropriate prescribing and monitoring of oral nutrition supplements within CLCH.

To maintain accurate documentation in dietetic records, medical notes and nursing notes in line with department and Trust guidelines.

To make recommendations for the appropriate prescription of oral nutrition supplements in line with NCL Formulary.

To be responsible for organising own clinical work load, administrative tasks and to be flexible to the needs of the service as indicated by the line manager.

To be responsible for inputting clinical activity data into the activity reporting database and generating reports on a regular basis.

To liaise with Dietitians and other health care professionals to ensure clients discharged from secondary care are followed up appropriately and receive ongoing input as required following discharge.

To provide education and training sessions to internal and external key stakeholders, as required by the service.

To participate in clinical governance activities within the Nutrition and Dietetics Service and complete required competencies for progression.
