Didsbury Medical Centre is a busy, innovative, training GP Practice which is conveniently situated on the edge of Didsbury Village and in the heart of the local community.

We are looking for an enthusiastic and motivated individual to join our friendly, forward thinking multi-disciplinary team. This is a new position in Primary Care and an integral part of our proactive care for patients with long term conditions. We are looking for someone with excellent communication skills, is organised and efficient and can work well within a team or on their own initiative.

The Care Coordinator admin role is an important new role in Primary Care. You will be responsible for using multiple software packages to coordinate and recall patients for their annual reviews, run searches, contact patients by telephone or text message, ensuring patients are booked into the most appropriate review appointments and the correct checks are carried out by clinicians. Experience in Primary Care is preferable, although not necessary.

**Job Types**: Full-time, Permanent

**Salary**: £22,269.00 per year
