These Transitions posts provide an exciting opportunity to work with care experienced young people as part of an initial 2-year contract funded by the Department for Education under a Staying Close project, which Derby City successfully bid for to deliver a Staying Close model of supported accommodation for young people transitioning from residential care to independent living.

As part of the Staying Close project the successful applicants will be expected to demonstrate that they have a working knowledge and experience of directly supporting vulnerable young people and be able to deliver direct 1-1 sessions covering independence and life skills to care experienced young people. You will be providing practical, emotional, social, financial advice and support to young people both preparing to leave care and those who have left care to enable them to make a successful transition to adulthood and independence. Transitions workers will work exclusively with a small group of young people on the Staying Close project.

Joining us as Transitions Workers, we try to provide everything you need to feel happy that you’re doing the best possible job. Working alongside your Team Manager, Leaving Care Personal Advisors, Social Workers within the Children in Care Service and multi-agency professionals, you will contribute to young people’s focussed and co-produced plans of support which will inform the young person’s pathway plan, ensuring that appropriate services are provided to ensure the most positive outcomes for vulnerable children and young adults.

The Children in Care and Leaving Care Service is committed to ensuring our Corporate Parenting obligations are met along with our Pledge to Children in Care and Care Leavers. Derby has a vibrant Children in Care Council and care leavers forum. We expect that the voice of our children and young people is embedded into their Pathway plans and considered when decisions are made which will affect them. The co production of the decisions and plans for young people are an essential part of this role.
