**Frailty & Phlebotomy Care Coordinator**

**Working pattern**: Full-time, 37.5 hours per week (of which a minimum of 6 hours must be worked on Saturdays)

**Pay**:Up to £12.50 per hour weekdays /£18.75 per hour Saturday depending on experience

**Job summary**

One of the main aims of this role will be to help to support individuals to manage their health and wellbeing in their own homes for as long as possible through the provision of general health care checks and a phlebotomy service.

The Frailty and Phlebotomy Care Coordinator will deliver a phlebotomy service and general healthcare assessments for housebound patients across 5 practices on behalf of North Oxfordshire Rural Alliance (NORA) PCN. This will involve assessing required equipment and consumables from one of our practices, delivering the phlebotomy service and general healthcare checks in the patient’s own home and liaising with the patient’s GP if there are any health concerns.

The post holder will provide an innovative, integrated service to anyone who has been identified as housebound or at risk of frailty. The role requires a caring, dedicated, reliable and patient-focused individual who enjoys meeting new people.

The post holder will be expected to:

- Work closely with practice teams to coordinate the delivery of phlebotomy and general healthcare services to housebound patients across the PCN
- Undertake basic assessments such as taking weight, BP and pulse rate to ensure that any housebound patient seen is receiving sufficient care and to contribute to the management of their long-term care.
- Deal with a vulnerable group of patients with the potential requirement to highlight those who have limited social/health support and to signpost them to available community services
- Learn the roles of the different community health and social care professionals to connect patients to available community services in order to obtain the appropriate care and support they require.

To be successful in this role you will need to have a strong track record in providing excellent general health care and phlebotomy services. Previous experience of care in the community is not essential but is an advantage. This is a developing role and so there will be the opportunity to shape this role to suit the needs of the community.

The North Oxfordshire Rural Alliance (NORA) is a friendly, supportive and caring Primary Care Network covering approximately 47,000 patients. NORA includes 5 GP practices in North and West Oxfordshire: Bloxham & Hook Norton, Chipping Norton, Cropredy, Deddington and Wychwood.

NORA practices operate as independent units but also work closely with each other to facilitate the efficient management of the PCN by using standard IT, Training and Clinical policies and procedures.

While we are happy to consider flexible working, this role will require the successful applicant to work the majority of the hours across the normal working with a minimum of 6 hours on a Saturday.

As the Frailty/Phlebotomist Care Coordination role is required to visit patients in their own homes across a wide area of North Oxfordshire, it is important that you have your own vehicle in full road-worthy condition and a clean UK driving licence. You will be reimbursed for your travel expenses.

**Main duties of the job**

You will be joining a growing PCN Clinical team as the lone Frailty/Phlebotomist Care Coordinator under the supervision of the Nursing Services Manager based at Chipping Norton

You will provide the following support to housebound patients based within the NORA Primary Care Network (PCN) catchment area.

Phlebotomy
- Using technical equipment to safely draw blood from patients
- Explaining the process of drawing blood clearly to a patient
- Answering patient questions regarding collection techniques and offering additional resources on the collection process, if the patient requests them
- Reassuring and calming patients who might be nervous or scared of the procedure
- Ensuring that the patient is as comfortable and relaxed as possible and confirming their agreement to the collection of a blood sample
- Checking patient identification against GP/Nurse request and confirming with patient whenever possible
- Accurate labelling of samples to confirm patient identity
- Sending blood samples to testing laboratories
- Ensure that domiciliary blood samples are returned to the hub in time for courier collection and on Saturdays, ensure that domiciliary blood samples are taken to the lab at the Horton Hospital at the end of the shift.

- Identify frail vulnerable groups of patients in the community to assess whether there are any unmet health
