We are delighted to announce our partnership with Cardiff and Vale UHB that will commence from the 1st April.

We will be making our Supported Accommodation available to our Health colleagues who have identified the need for this to be a key stage for their patients in returning to live in the community. Not only will this ease the pressures on the UHB but more importantly will be a stepping stone for patients to resume their best lives living independently.

Cardiff Mind has previously had a strong relationship with Cardiff Council in delivering similar services and it is this strong track record together with our reputation in providing services to the citizens of Cardiff that has brought about this exciting service development.

This initiative is a perfect example of the way in which Health, the Local Authority and the Third Sector can work collaboratively to improve lives of people who are experiencing difficulties with their mental health.

To deliver this new service we are actively recruiting for Support Workers both full time and part time and would be delighted to hear from anyone who wishes to make a difference.

**Job Title**: Mental Health Housing Support Worker

**Responsible to**: Supported Accommodation Manager

**Responsible for**: A broad range of support duties within Supported Accommodation, Domiciliary and Tenancy Support services

**Job Purpose**: To provide assessment and develop support plans for service users. To co-ordinate and provide support to people living in supported accommodation and their own homes in line with the plans. To work with service users to achieve positive outcomes that will enable them to take greater control of their own lives.

**Core Duties and Responsibilities**

**1. Assessment and Review of Service Users needs**

To participate in the assessment of referrals into the Supported Accommodation project/ Floating Support Scheme and work with applicants to identify the goals they wish to achieve in line with the objectives of the scheme.

To work in partnership with the Service User to plan ways in which these goals will be met and develop and regularly review the progress made in delivering the outcomes identified within the plans.

To work with the service user alongside staff from Cardiff Mind, Volunteers, and other agencies to enable the tenants to achieve the goals they have identified in their Individual Support Plans.

**2. Access to Information**

To ensure that the Service user can access information not only on services available in Cardiff and issues relating to the needs of people with mental health problems, but also to general information that will enable them to make informed decisions and fully participate as citizens.

**3. Service User Involvement**

To facilitate the development of the ability and potential of each service user to become involved in the development and review of their Support Plan. To encourage active service user participation in influencing services provided by Cardiff Mind and in the wider sphere of mental health service provision in Cardiff.

**4. Organisational Issues**

To work in accordance with the organisational policies and procedures.

To actively participate in supervision, performance review and training provided by Cardiff Mind and identified as adding value to the service.

**5. Housing Management**

To work in line with the requirements set by the Housing Associations to ensure a good standard of housing is provided to service users.

**1. Assessment and Review of Service Users’ needs**

1.1 To carry out a full assessment of all persons referred to the scheme as directed by the Manager of the Service.

1.2 To prepare individual support plans for persons joining the scheme.

1.3 To carry out risk assessments of supported persons and the environment in which the support will be delivered as required.

1.4 To plan and carry out reviews of all support plans in compliance with the requirements of the standards and timescales for the service.

1.5 To be the Key Worker responsible for assessment and review for named individuals.

**2. Co-ordination and implementation of Support Plans.**

2.1 To co-ordinate work with the service user and other staff on the goals identified by the in support plans, including personally delivering those elements for which you are responsible or parts of that work as appropriate.

2.3 To record the work carried out with the service user to evidence progress and facilitate effective evaluation of the support plan.

2.4 To provide appropriate emotional support to enable the tenant to develop new skills and achieve identified goals.

2.5 To develop with the service user a range of options which will open the opportunities available to increase their expectations and make an informed choice, including expanding social networks.

2.6 To work as part of a team to offer consistency in the quality and approach of the support offered.
