Healthy Child Programme Health Mentor

**The post holder will be required to have**: Communication and Relationship skills 1. To work pro-actively to build relationships with relevant agencies to engage children and young people who would benefit from the service 2. To liaise with young people, parents and community health staff, School staff or other services working with children and young people to support the delivery of effective referral and recruitment pathways. 3.
To provide information on the programme in a range of formats which are user-friendly to encourage self-referral. 4. To show tactfulness and sensitivity, negotiation, motivational skills to identify and develop effective pathways for lifestyle change with children, young people and families. 5.
To guide and support individuals to undertake health risk assessments, recognise key areas for change and set effective lifestyle goals. 6. To deliver brief interventions for health improvement to individuals on the health mentor programme. 7.
To develop healthcare plans and support individuals to meet their goals. 8. To encourage and support self-motivation in participants sustaining lifestyle changes. 9.
To measure and report on the outcomes of healthcare plans. 10. To plan and deliver health improvement group interventions. 11.
To promote the Health Mentor Programme and other lifestyle services to current and potential participants and other agencies. 12. To work with other mentoring and early intervention services to ensure seamless support for children, young people and families. Analytical and Judgement Skills 1.
To analyse the results and explain to children, young people and families the complex information based on the results of their health and wellbeing assessment. 2. To assess the readiness of children, young people and families to change lifestyle behaviour 3. To provide an appropriate package of lifestyle support with the child or young person (and family as appropriate) to include: - A health behaviour check - Choosing a health behaviour to change - Planning behaviour change - Mutually agreeing a personal health plan - Reviewing behaviour change 4.
Maintaining behaviour change 5. To use motivational interviewing/behaviour change techniques to support service users to determine an action plan based on the range of options available for them. 6. To recognise and refer complex cases to the band 6 school nurse or appropriate professional.
7. To respond professionally with enquiries from professionals and other agencies and the community. Planning and Organisational skills 1. To ensure electronic diary is utilised effectively through prioritising, planning, organising.
2. To plan, organise and deliver health mentor brief intervention, care plans and assessments at local venues based on guidelines and protocols. 3. To ensure service users are appropriately signposted into a range of local activities and opportunities at times and venues that meet their needs.
4. To actively contribute to the development of the Health mentor service implementation plan. EFFORT AND ENVIRONMENT The role requires the following effort and environment: 1. Ability to use IT and office equipment effectively within scope of role.
2. Use of own car in order to travel around Tameside as required. 3. Carrying equipment to and from venues.
4. Ability to handle situations sensitively and calmly when children and young people become distressed. 5. Periods of concentration required when working with service users in either 1-1 or group situations.
6. Ability to capture data on relevant systems.
