An exciting opportunity has arisen for a discharge co-ordinator to join the Surgical Emergency Unit based within the John Radcliffe Hospital.

The Discharge Co-ordinator will act as a central point for all enquiries relating to discharge planning within the ward. They will work closely with the Ward Co-ordinator, the SEU bleep holder and the Divisional Bed Manager. They will also work closely with all members of the Multi-disciplinary Team to support the safe, appropriate, and timely discharge of patients. They will communicate effectively with the patients and their friends and family to ensure they are actively involved in the discharge process so that their personal preferences are being considered and met wherever possible. The successful applicant will have a positive and professional attitude, excellent communication skills and able to work under their own initiative.

Informal visits encouraged.
