The Forensic Outreach and Liaison Service (FOLS), is a community-based service operating across Sussex, providing holistic support to mentally disordered offenders.

We are currently seeking a full-time Band 4 Nursing Associate, to help us deliver bespoke physical and mental health well-being interventions. We will support you to work independently, under the guidance/direction of a Registered Nurse, to deliver a range of clinical activities in line with patients' individual care plans.

As a Band 4 Nursing Associate, you will have completed your Nursing Associate training and be registered with the NMC.

Your main role within our team will be to provide a range of physical and mental health well-being assessments, activities and interventions to a specific group of patients under the care of the service, across Sussex. You will be working with patients with offending behaviours, including those under probation and on mental health act restriction orders under the Ministry of Justice.

You will work alongside Registered Nurses and other members of our multidisciplinary team, to deliver care across Sussex. You will be assigned a named mentor from within the team who will meet with you on a regular basis to support you clinically and professionally.

You'll need to be able to travel across the county, but we can be flexible with your work base in Sussex, as we currently have two bases - one at Hellingly (East Sussex) and the other in Worthing) West Sussex).

**Important info**:

- FOLS operational hours: Monday to Friday, 09.00 - 17.00.
- This post is 37.5hrs per week.
- Pay Band: AFC Band 4 (travel expenses are paid according to AFC)
- Base in East or West Sussex (see above)

Forensic Outreach Liaison Service (FOLS)

**Main Responsibilities**:
As a Registered Nursing Associate within the Forensic Outreach and Liaison Service (FOLS) your main role will be to provide outstanding, evidence-based care to patients under our service. You will take a lead in offering and delivering physical health clinics, including health and well-being assessments, health promotion interventions, as well as assisting patients with activities of daily living and personal care, and medication administration/concordance as needed. You will liaise with the individual, their care team and carers as appropriate, to really listen and understand the individual's needs, ensuring that each package of care offered is truly personalised.

Another important aspect of the role will be to work closely with internal and external partner agencies to forge strong links and working relationships, and to increase knowledge of local services/well-being interventions/resources and access to them, for patients under our care in different areas across Sussex.

We would also encourage you to build on your own skills and knowledge, by keeping up-to-date with relevant training, but also to help us deliver the best possible service we can by actively engaging in quality improvement initiatives, and by sharing ideas and learning.
