Embark on a journey of opportunity This service involves the rehabilitation of people across the whole of North Somerset, providing a golden opportunity for you to support individuals in regaining their skills and confidence following discharge from a hospital or other healthcare setting.

This is usually for a period of up to 6 weeks, supporting people to learn or re-learn the necessary skills to manage their day-to-day living tasks again following an injury or illness and wherever possible achieve a swift return to their independence.

Your role as a Rehabilitation Care Practitioner is a stepping stone to career progression, focusing on providing support and reablement-focused care to enable people to confidently live independently. Put all possible measures in place to make achieving goals a reality.

Collaborate with Health & Social Care professionals from various sectors and backgrounds. Dive into the action with our experienced Rehabilitation Care Practitioners and Coordinators, crafting first-class reablement plans for each individual.
