Tasks and responsibilities Take referrals from practice staff (we use a system called Elemental for this) Assess young people for psychological and social support Provide information and support to the young person and their family on social interventions that could improve their health and social wellbeing Support clinical staff in identifying younger patients in need of psychological and social support and what support might help Work with other young peoples agencies in our neighbourhood, including school services, child and adolescent mental health services, third sector child and adolescent services and social services Work with other social prescribers in the network Participating in multidisciplinary meetings for younger patients with complex needs Administration Fully document all aspects of patient care and complete all required paperwork for legal and administrative purposes in accordance with relevant standards Work in accordance with internal administrative systems relating to but not limited to the management of clinical data, Send and receive written information on behalf of the practice relating to the physical and social welfare of patients Work closely with other clinical staff and administrative managers in the setting up and improving practice services for the care of children and young people. Health and Safety Comply with the practice health and safety policies by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System Comply with the Data Protection Act (1984) and the Access to Health Records Act (1990).
