- Coordinate and manage the care of patients, ensuring their needs are met and their care is delivered in a timely and efficient manner.
- Serve as a liaison between patients, healthcare providers, and insurance companies to facilitate communication and ensure appropriate care coordination.
- Conduct assessments of patients' needs and develop individualized care plans.
- Monitor patients' progress and make adjustments to care plans as needed.
- Schedule appointments and coordinate transportation for patients as necessary.
- Provide education and support to patients and their families regarding their healthcare options and resources.
- Maintain accurate and up-to-date documentation of patient interactions and care plans.

**Qualifications**:

- High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred.
- Previous experience in a healthcare or social services setting is highly desirable.
- Strong organizational skills with the ability to prioritize tasks and manage multiple responsibilities simultaneously.
- Excellent communication skills, both written and verbal, with the ability to effectively interact with patients, healthcare providers, and insurance companies.
- Ability to maintain confidentiality of patient information in accordance with HIPAA regulations.
