Care Coordinator

Gillespie Recruitment Ltd is seeking a highly skilled Care Coordinator to join our client's team in the North East. As a Care Coordinator, you will play a vital role in providing essential support to patients, assisting with daily tasks, and ensuring their well-being.
Key Responsibilities:- Provide exceptional support to patients, including personal hygiene, feeding, and mobility assistance.
- Supervise the day-to-day operational care delivery within the service, ensuring that Service Users are at the heart of the care delivery.
- Plan and allocate care packages to appropriate care workers, matching clients' needs with the skills and attributes of the care support staff.
- Liaise with key stakeholders, including social workers, police, GPs, nurses, ambulance, pharmacists, and physios, to develop and deliver person-centered packages of care.
- Create, monitor, and maintain safe rotas for all staff, ensuring all packages of care are covered and that the correct hours are allocated.
- Responsible for ensuring safe levels of cover are in place while staff take annual leave or cover short/long-term sickness.
We are looking for a motivated Care Coordinator with experience in Domiciliary Care. If you have a strong organizational skills, the ability to prioritize tasks effectively, and a passion for delivering high-quality care, we want to hear from you.
- Health and social care qualification, such as a Level 2 or 3 Diploma in Health and Social Care.
- Strong organizational skills with the ability to prioritize tasks effectively.
- Ability to drive.
- Proficient in using office software, such as Microsoft Word, Excel, and Outlook.
- Detail-oriented with a high level of accuracy in record-keeping.
- Ability to work well in a team environment and collaborate with others.
- Knowledge of medical terms and healthcare processes is preferred but not required.
