We are looking to recruit to a new role of a Patient Pathway Navigator to join our newly developed community and specialist Virtual wards, in collaboration with partners across health and social care within the East Cheshire region.

The role of a Patient Pathway Navigator is to work closely with the Clinical Manager to ensure that referrals to community specialty virtual wards are processed in a timely and efficient manner in line with nationally defined and locally agreed targets and to facilitate an effective patient journey by coordinating a recommended plan of care, working closely with the Transfer of Care Hub, Frailty Team and Community Teams.

**Hours per week 37.50
