The Diabetes Care For You service is a multi-disciplinary team that includes Consultants, Diabetes Specialist Nurses, Dietitians, Podiatrists and Psychotherapists. Its aim is to help to improve health outcomes for people at risk of diabetes or who have a diagnosis of diabetes, and will meet the needs of the growing diabetes population across Brighton and Hove, and High Weald, Lewes and the Havens, both by providing interventions to people with diabetes and increasing the level of skills and knowledge within primary care.
The goal of the service is to enable patients to access education, information, advice and support to enable them to make informed choices about how to manage their condition, so that they can stay as well as possible for as long as possible, and reduce their risk of diabetes related complications. Joint care planning and goal setting will be a core focus of the new delivery model, with patients in control and empowered to self-manage.
The post holder will be responsible for the provision of Podiatry services to a varied client group, providing a high standard of clinical care.
The post holder will also support other colleagues and case loads as deemed necessary by the manager of the service in response to local needs at the time.
The post holder will also need to develop individualised care programmes for patients with a wide variety of clinical needs to the direction of the Specialist Lead Podiatrists. This will include some high-risk patients but the post-holder will be supported by a more senior clinician whilst undertaking treatments. Cases may include those with acute or chronic neuropathies, chronic diseases, vascular problems and basic wound management.
The post holder will be expected to undertake home visits for housebound patients. Patients will be screened to ensure that they are not highly complex cases.
The post holder will receive professional support from Specialist colleagues and the Podiatry Services Management Team.
Although the role is based in the specialist Diabetes service there will be support and opportunities to undertake rotations through nail surgery and MSK.

To provide Podiatry treatment for a variety of patients within the defined criteria for the service.
To be an autonomous practitioner, professionally and legally accountable for all aspects of your clinical work.
To be responsible for the assessment, diagnosis, planning, implementation and evaluation of podiatric care to for a variety of patients within the service, presenting with a wide range of clinical needs, to include some high risk groups such as those with: Diabetes, Chronic Neuropathy, Chronic disease e.g. Rheumatoid Arthritis, Vascular problems, Basic wound management/Tissue Viability needs
New graduates will be closely supported by a Specialist Lead Podiatrist whilst working in clinic and will undertake rotations within the department to gain knowledge and skills within specialist areas of Podiatry.
After a period of orientation within the department, the post holder will be responsible for the assessment of some of the new patients referred to the department within the boundaries specified by the service specification. This will involve developing care packages and the provision of appropriate foot care education advice. New patients will be screened by a senior clinician for suitability and the care packages will have input from a senior colleague.
To achieve and maintain the relevant competencies to fulfil the requirements of the trust Foot Protection Team [usually to level of B5 clinician in this role]
To provide domiciliary care for those patients that have already been assessed as appropriate within the service criteria.
To liaise with other professional disciplines and senior colleagues in the department, to ensure appropriate service delivery. To influence appropriate referrals for the client group, and where possible to improve the effectiveness and efficiency of service delivery.
