We have vacancies for a Social Prescribing Link Worker, the position will be initially for a period of 6 months and dependant on funding will hopefully be extended. As an organisation we have been delivering services with thi srole for the past 4 years.

Vale of Clwyd Mind (VOCM) is affiliated to National Mind, the leading mental health charity in both Wales and England. We were established in 1979 and since that time we have worked with and supported numerous individuals experiencing the widest range of mental distress. This has been achieved through its housing, social opportunities, employment, rural outreach and training projects. VOCM has built up a matchless reputation in North Wales as a first class, user-informed provider of services. We have a history of successful partnership working with the voluntary sector, private companies and statutory organisations in order to provide services in the most effective and efficient way.

**Job summary**

The Link Worker will support the delivery of Social Prescribing in rural Denbighshire. They will meet clients referred by primary care and help them to access more appropriate services and support like welfare services, well-being activities and non-clinical interventions. The focus will be on empowering the client to make their own choices and to set personal goals. The Link Worker will need to build good relationships with their local primary care teams, as well as develop an in-depth knowledge of local services and activities. In some circumstances the Link Worker may also be required to assist clients who self-refer or help clients who are referred internally or by other organisation.

The Link Worker will support the collection of client information so that the effectiveness of the service can be evaluated. The link worker will also be involved in the design phase, working collaboratively with partners to complete detailed mapping work of services and referral pathways.

The Link Worker will be expected to travel between sites in Denbighshire on a regular basis via our specially adapted vehicle DORIS (Denbighshire Outreach Rural Information Service)

**MAIN DUTIES**

**CLIENTS**

1. Create a safe and trusting environment for clients.

2. Use various techniques to assess clients’ needs such as the 'What Matters’ approach and motivational interviewing.

3. Work with clients to help them identify their support needs and link them with local services/activities that could meet those needs.

4. Help and empower clients to complete goal setting exercises.

5. Support clients to access services through various mechanisms such as coaching, making onward referrals, providing information, or arranging for peer support and assistance.

6. Work with clients to review progress and adjust plans as required.

7. Assist clients to complete evaluation surveys.

**PARTNERSHIPS**

8. Build strong relationships with local primary care team.

9. Receive referrals from primary care team and manage own schedule of client appointments, including initial meetings and client follow-ups.

10. Develop and maintain a comprehensive menu of local services and activities and identify local gaps in service provision.

11. Build relationships with local service providers for the purpose of maintaining up
- to-date knowledge of local service offers and waiting times.

12. Take an active part in the development of the service model and evaluation.

**MONITORING AND EVALUATION**

13. Ensure that all monitoring, reporting, and evaluation forms are completed on time and accurately and are securely stored in accordance with local GDPR requirements.

14. Support all project evaluation activities as required.

15. Regularly report to line-manager and Board on progress of service.

16. Work across multiple local sites as required.

17. Participate in wider Vale of Clwyd Mind team meetings and supervision.

18. Work with and support the development and coordination of volunteers.

19. Work flexibly as specified in contract.
