We have an exciting opportunity for a passionate Nurse/OT/Social Worker with a specialist interest in Autism to join the Amber Valley Community Mental Health Team.This is a new role, and our aim is to expand support to adults with Autism and Mental Health needs within the CMHT. The post holder will be pivotal in developing this role and evidencing the benefits with a view to future changes within our community services as part of a pilot/proof of concept.

We are looking for individuals who share our strong, value-based and person-centred approach, and have a keen interest in and understanding of the support needs of people with Autism to help develop, shape and deliver this brand new service.

The post offers an opportunity for you to develop into the clinician you aspire to be; with training, supervision, and an ethos promoting innovative practice. We are proud of our MDT approach to healthcare and the value we place on each staff member’s unique knowledge and skills.

You will work with people with Autism and Mental Health needs to support them and other practitioners within the team to provide appropriate support and interventions avoiding people going into crisis and support in avoiding admission alongside our CRHT (Crisis Resolution and Home Treatment) and SAT (Specialist Autism Team) services.

You will promote person-centred practice and work creatively to improve people’s quality of life.

The work is mainly community based with some in-reach work into hospitals when required.

As a Community Autism Practitioner in the team you willprovide a vital role in assessing the care needs and risks of referred individuals and create, implement and evaluate treatment plans and bespoke interventions with a person-centred focus. You will also provide support to other practitioners within the team.

You will lead and develop the team by sharing your knowledge and skills to create a truly integrated approach to care.

The Trust is working closely together and in collaboration with the wider Joined Up Care Derbyshire organisations.

You will work closely with partner agencies, the voluntary sector to ensure a holistic approach to service user care and positive outcomes across all areas of individuals’ lives.

The working hours are 9am-5pm, Mon-Fri.
