Improvement Facilitator

MAIN DUTIES AND RESPONSIBILITIES Supporting Quality Improvement in divisions Acting as divisional improvement facilitator for assigned division(s). Supporting divisional management team to identify local improvement priorities for targeted QI activity. Support division to establish and run the divisional QI governance process for targeted QI. Assisting with the identification, scoping, resourcing and planning of targeted QI projects.
Provide information and reports on the status of divisional QI projects which have been registered on the LIFEQI system and on the status of QI training within the division. Support development of divisional improvement capability development plans. Providing local coaching, training and facilitation support for divisional teams that are running QI projects, including Quality Councils. Support teams & services to establish local QI processes.
Responsible for maintaining effective relationships with identified sites, ensuring QI maintains a high profile and overcoming challenges as they arise. Helping teams to reflect on current working practices and to challenge and encourage changes to behaviour and practice. Contribute to the overall development of the QI approach at CLCH. Delivering improvement training Responsible for devising and delivering on-site training for teams in quality and service improvement, in line with the Trusts approved improvement methodology.
**These may include**: problem definition, PDSA, root cause analysis, process mapping, management of flow, understanding and reacting to data, understanding capacity and demand, lean methodologies, stakeholder engagement, involving patients, influencing behavioural change and improvement project management. Providing high quality training in quality and service improvement methods as part of the continuous quality and service improvement programme. Devising, delivering & facilitating workshops/programmes/events to share good practice and experiences of improving services. Raise the profile and support the development of the QI website.
Provide staff with useful links to resources to assist with improvement, initiate and contribute to active discussions and contribute resources to help maintain an up to date repository of best practice. Maintain accurate and complete information on training delivery and attendance, liaising with CLCH Academy as necessary to ensure learning is recorded. Evaluating training performance and taking proactive action improve delivery, learning outcomes and impact. Ensure effective use of training resources, minimising costs and avoiding unnecessary waste.
Maintain effective relationships with key stakeholders, maintaining commitment and influencing change at all levels without having direct management responsibility for service delivery. Support the Trust-wide annual clinical audit process, providing support with quality and service improvement methods to audit leads, ensuring achievement of compliance with identified standards. Support the Trust-wide clinical outcomes programme, providing teams and services with expertise and support in identifying issues and opportunities for improving clinical outcomes and supporting them to undertake and test ideas using robust methods. Other tasks and duties Take direct responsibility for the planning, implementation and delivery of discrete projects, as required.
Creating and maintaining all associated project related documentation as required by the Trusts project management policy. Ensure delivery of project objectives on time, within budget, to an agreed quality, identifying and managing risks and issues as appropriate. Liaising with clinical services and directorate management teams to identify improvement opportunities, assessing and recommending those most beneficial in relation to relevant identified aims. To take an active part in meetings, workshops and presentations for a wide variety of audiences, demonstrating consistently high qualities of communication and persuasiveness skills.
To analyse data from various sources, interpret / evaluate to propose a range of options, conveying complex information in a simple and understandable manner. To maintain an up to date knowledge of best practice as well as local and national policies regarding improvement. To manage own workload and priorities and re-negotiate as appropriate. To be proactive in gathering agreed information.
To take the initiative in proposing methodologies, approaches and technologies within own area of expertise. To be responsible for delegated budgets, procuring and managing resources as required to support delivery of identified work streams. The above indicates the main duties of the post which may be reviewed in the light of experience and development within the service. Any review will be undertaken in conjunction with the post holder.
