To obtain the services of allied healthcare professionals and support staff as requested by nursing/medical staff (e.g. Phlebotomists/ECG Technicians/Radiographers/Porters).

**In respect of outpatient referrals**:
To make referrals to outpatient clinics elsewhere in the Trust as requested by nursing/ medical staff, completing all documentation required for the referral

To ensure the availability and preparation of patient hospital notes together with the relevant documentation for all patients admitted to the service. To ensure the availability of patient hospital notes when requested for clinical audit/Risk & Governance departments.

To use the case note tracking system to record the location of case notes, ensuring that the tracking system is maintained and that staff with access to it can locate case notes over a 24 hour

To liaise with medical secretaries and other hospitals to locate missing case notes and if unsuccessful to escalate to Operational/Clinical

To complete the electronic discharged

As instructed by a nurse or doctor, to arrange for any necessary follow-up investigations for

To collate documentation and file appropriately in hospital case

To book patient transport when

To work as part of the wider ward team assisting with general

To answer the telephone and door in a professional and courteous manner.

To assist relatives/visitors as per unit guidelines and Trust policy.
