Working as a PCN Social Prescribing Link Worker Lead offers a unique opportunity to make a meaningful difference in the lives of patients by addressing social, emotional, and practical needs through non-medical interventions. By leading a team of dedicated link workers and collaborating with various stakeholders, you will contribute to the development of a holistic and person-centred approach to healthcare within Mid Chiltern PCN.

We are looking to recruit a Lead to manage our small team of link workers, who will ideally be someone with experience of working in the community as a social prescriber, but we would be willing to consider potential individuals with the right mix of skills looking to transfer into a new career area.

The post holder will be responsible for delivering social prescribing and developing the operational aspects of the service across the five practices which make up Mid-Chiltern PCN.

As well as providing leadership to the Social Prescribing team the post holder will manage a caseload of clients through assessment to onward referral, working with clients in the practice that have been referred by GPs, practice staff and staff working in the community

Provide effective leadership to a team of social prescribing link workers, including recruitment, training, and performance management.

Foster a positive and collaborative team culture, encouraging innovation, creativity, and professional growth.

Support link workers in their day-to-day activities, providing guidance and supervision as needed.

Develop and implement a comprehensive social prescribing program in collaboration with healthcare professionals and community partners.

Establish and maintain strong relationships with healthcare providers, community organisations, and local authorities to promote the benefits of social prescribing.

Collaborate with PCN leadership and other managers to integrate social prescribing into the wider healthcare strategy.

Act as a liaison between the PCN, link workers, and external stakeholders, facilitating effective communication and partnership working.

Ensure adherence to relevant policies, procedures, and guidelines in the delivery of social prescribing services.

Monitor the quality and safety of interventions, identifying and addressing any concerns or risks promptly.

Stay updated with the latest developments in social prescribing and related fields, incorporating best practices into the program.

Establish data collection and reporting mechanisms to monitor outcomes and demonstrate the impact of social prescribing interventions.

This is an exciting opportunity to join Mid Chiltern PCN which was set up on 1st July 2019. Its population is approximately 43,700 patients made up of 5 Practices (Amersham Health Centre, Hughenden Valley and Chequers Surgeries, John Hampden Surgery, Rectory Meadow Surgery and The Prospect House Surgery).

**What experience will you need?**
- We are looking for someone with an understanding of social prescribing.
- You'll be able to demonstrate the ability to be a good listener, have time for people and be committed to supporting local communities to care for each other.
- You'll have experience of working positively with people facing complex social and emotional challenges.
- You'll have great interpersonal skills and a can-do approach to supporting people, community groups and local organisations.
- A car is essential and a knowledge of the local area would be ideal as some driving is involved between the five surgeries and also attending local community events.
- It is important you are I.T. literate as we use multiple clinical systems.
