**About the Role**:
A fantastic opportunity has arisen for a GP assistant (GPA) to join our team at North Uttlesford PCN. The GPA position is a support role to assist local GPs in identifying the needs of our Frail patients in the community.

The post-holder will form part of our multi-disciplinary, primary-care team and contribute to the smooth running of the PCN. The main duties of this role will be do review patients on the Frailty and Dementia registers and identify those that need further input from the GP.

We are looking for a full time role, but would accept part-time applicants also.

This post will involve driving out to review patients in their homes, assessing their care needs and falls risk as well as their frailty status and signposting them to relevant support agencies if needed.

The post holder will be supported by a GP educator in the PCN team and we hope will be able to obtain 10 credits and level 4 in the Health education England GPA programme.

- Administrative experience;
- Strong organisational skills;
- IT skills
- Excellent communication skills and a friendly manner;
- Ability to understand the need to maintain confidentiality and data security, at all times;
- Ability to multi-task and to work accurately under pressure;
- Ability to work in a team as well as being comfortable working independently.
- Cleaning driving licence and a car required.

**About Us**: Our Primary care network (PCN) is based on 4 North Uttlesford Practices working together. PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home. Clinicians describe this as a change from reactively providing appointments to proactively caring for the people and communities they serve.
We are a growing, supportive team of staff led by our Clinical Director Dr Richard Boyce, GP and Nurse educators, 3 Pharmacists, Paramedics, health and wellbeing coaches, an integrated mental health team and social prescribers.
