This is a new exciting permanent job opportunity to join as the Eye Clinic Liaison Officer at UHCW. If you are someone with recent proven /evidence based experience of person centre working or delivering information & advice in a health, social or community setting this role may be just right for you.

You will be expected to have a close working relationship with a wide spectrum of senior Ophthalmology staff and other colleagues across both the University Hospital Coventry and Warwickshire and St Cross Hospital Rugby as well as integrated care systems and local authorities. The role is to provide a supportive service to visually impaired patients of all ages, their carers and their families, through the provision of information and emotional support, helping them to make decisions about their future needs.

**Key aspects to the role are**:
To provide emotional support to patients of all ages, families, and carers, ensuring that adequate information is given and providing follow up if required.

To receive direct referrals from Consultant Ophthalmologists, Eye Clinic nursing staff, Orthoptists, Optometrists and Low vision staff and self referrals from patients, families and carers.

To discuss and explain registration in detail.

To support patients with their agreement throughout the registration process.

To assist Consultants to complete/process the necessary paperwork (Certificate of Visual Impairment / CVI) in accordance with the Department of Health guidelines. This includes the provision of essential data for the Public Health Outcomes Framework PHOF (preventable sight loss indicators) and keeping local records of registrations.

To support patients following diagnosis who do not wish to be registered or who are not suitable for registration.

The role requires excellent communication skills with all age groups both verbally and in writing and have empathy with others respecting their individual dignity.

You must have the ability to work as part of a team and in partnership with other service stakeholders and demonstrate initiative.

Assess, plan and ensure patients receive prompt care according to their needs.

To ensure that patients who are experiencing anxiety, distress and/or practical difficulties due to their eye condition, irrespective of their registration status, are referred to the Vision Support services.

To liaise with the Low Vision Clinic and Stroke Orthoptist and ensure patients are correctly referred to the respective services.

To maintain a specialist knowledge base of related fields/ resources for visually impaired patients.

To provide written information as required for the benefit of visually impaired patients, within the Trust.

To provide information regarding statutory and voluntary agencies

To forge links and work closely with both the Health and Social Care Support Networks and Local Authorities in order to develop continuity of the service.

To ensure prompt registration in accordance with National Standards.
