The post holder is responsible for managing a Flying Start caseload within a defined population. They will be required to follow the 4 principles which underpins the health visiting philosophy; The search for health needs, stimulation of the awareness of health needs, Influence policies affecting health and facilitation of health enhancing activities. Key Tasks and Responsibilities of the Post will include managing a clinical caseload of children aged 0-5 years. Working autonomously to assess, plan, implement and evaluate health care interventions to meet the health and related health needs of individuals, groups and communities

The post holder is responsible for managing a Flying Start caseload within a defined population following the 4 principles which underpins the health visiting philosophy. 1 The search for health needs 2.The stimulation of the awareness of health needs. 3.Influence policies affecting health. 4 The facilitation of health enhancing activities.

Key Tasks and Responsibilities of the Post will involve managing a clinical caseload of children aged 0-5 years. Work autonomously to assess, plan, implement and evaluate health care interventions to meet the health and related health needs of individuals, groups and communities.

This post is fixed term for 12 months to meet the needs of the service.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Essential
- NMC register
- SCPHN
- Nurse Prescriber or working towards
- Experience in primary care and multi agency working
- Knowledge and experience in child protection
- knowledge and experience in health visiting
- Evidence of continuing professional development
- Excellent team working and communication skills
- IT skills
