To provide excellent clinical leadership and coordination of clinical care for trauma patients who are being managed at home or who are admitted via Accident and Emergency and or Fracture Clinic.

This role is embedded within the trauma wards but will outreach to A&E and theatres to ensure that trauma patients including hip fracture patients move through their pathway in the best optimised health.

This role is about helping patients get better sooner, whilst being the conduit between a variety of disciplines to get the best outcomes for patients whilst achieving quality indicators and financial efficiencies for the organisation.

Clinical, Interpersonal and managerial skills are key to the success of this role as is the need to maintain credibility and flexibility as a competent practitioner in an ever-changing environment, while continuing to develop professionally in line with the demands of a responsive nursing service focussed on the needs of patients.

The ability to lead on complex situations which require analysis, interpretation and comparison.

- To ensure that each patient’s hospital episode is managed effectively and efficiently through excellent communication and interaction with relevant healthcare professionals,
- To ensure that planned care takes account of patients’ needs and preferences and is delivered in accordance with the person centre framework.
- To lead and embed the hip fracture programme which includes; orthogeriatric assessment, the rapid optimisation of fitness for surgery and scheduled timely for surgery whilst ensuring targets met for best practice tariff payments and CQUINs.
- To facilitate the trauma meetings to ensure patients’ fitness for surgery, avoid cancellations of operations; reduce unnecessary pre-operative starving and patients admitted for timely surgery.
- To assist in data collection pertaining to trauma presenting findings at departmental meetings.
- To provide clinical education for patients, relatives, nursing doctors and AHPS.
