**JOB PROFILE**

Profile Title: Home Support - Wrap around care

Pay**:£9.84 p/h**

Contract: Part Time and Full Time Hours available

**Closing Date: Open**

**JOB PURPOSE**:
The Home support worker’s role is to deliver good quality bespoke support service in the home. The service offers a socially focused, practical form of support with daily living tasks to enable older people to manage at home and maintain their independence.

It is more flexible than a simple cleaning or shopping service, offering a broader range of support, tailored to the individual’s need to enable independence.

**Main Duties**:
**This is non-CQC registered support in the home, as opposed to higher level personal care. Supporters are not expected to deliver personal/ medical care as part of this role.**
- To support clients with practical daily living tasks in their own home, encouraging them towards independence. This could include meal preparation, cleaning and laundry.
- To support clients in the wider community by eg: doing shopping or collecting prescriptions.
- To support clients to remain physically and mentally active by providing a range of activities / hobbies suitable for them to undertake in their own home.
- To assist clients who have recently been discharged from hospital by providing support suitable to their need.
- Providing timely administrative, financial and client related paperwork as appropriate.
- Prepared to have a flexible approach to work patterns and service delivery to ensure a client responsive service.

**SKILLS AND EXPERIENCE**

**Client and customer care**
- Communicate well with people (verbal/written)
- Deal appropriately with complaints, compliments and comments
- Use client's own equipment - including cleaning equipment
- Handle money in an accurate and honest manner
- Promote and share information relating to Age UK activities

**Community Working**
- Work with older people to promote and maintain their independence
- Undertake Lone working
- Maintain a safe, tidy and clean working environment
- Ensure attendance at relevant training meetings

**Personal Skills**
- Driver with access to own vehicle
- Maintain confidentiality /adhere to data protection
- Work as part of a team
- Effective organisation of own work, meet standards and timescales
- Commitment to the aims, vision and core values of the organisation
- To offer a flexible and 'can do' positive attitude
- To demonstrate a commitment to personal development
- To gain a knowledge of all policies and procedures and adhere to these
- to work in a flexible manner
- To demonstrate an understanding of the needs of older people and their supporters

**Job Types**: Full-time, Part-time
