**Community Connect**

Is an individual and community mental health & physical activities engagement programme. We offer a trauma sensitive, innovative, and strengths-based community response to isolation, loneliness, social exclusion, mental ill health, and physical inactivity.

The service offers a safe space for adults to feel heard and to talk about what is important to them. What makes us unique is that the service sits within a wider network of The Active Wellbeing Society programmes which means we can offer practical solutions with links to Share Shacks and food support.

We offer individualised support via an engagement strategy based on a combination of telephone, direct support and groups; working towards understanding what is important to the person so to co-create personalised support. We aim to increase levels of connection, belonging and movement.

**The Move and Connect Peer Support Worker**

Will play a critical role in engaging and supporting people to take an active role in their mental wellbeing through a combination of social, physical and emotional interventions.

Peer support workers will be involved in delivering one to one and group sessions based on elevating levels of loneliness, isolation, anxiety, depression, panic attacks and moods disorders alongside encouraging physical activity.

There will be a strong focus on empowering positive lifestyle choices - an approach that identifies and works towards removing barriers to prioritising self-care. A key part of the role is advocacy, service navigation, information and signposting in to community assets, specialist services and opportunities available.

**Main Duties**

1. Work alongside the patient through their journey to set and achieve their personal goals

2. Work with patients in a face to face, virtual and telephone-based way to offer support that is responding to their needs.

3. Support patients to access statutory support including psychological interventions.

4. Create safe spaces to challenge and encourage shifts in thinking in relation to positive lifestyle changes alongside working with patients to understand the barriers and solutions to improved self-care.

5. Develop and maintain a good working knowledge of community resources and partner organisations to signpost clients and to promote social inclusion.

6. Encourage patients to engage positively in the wider community.

7. Reduce levels of vulnerability and risk of abuse - working within the adult safeguarding principles of personalisation. Ensuring that the person is fully engaged and consulted throughout and that their wishes and views are central to any final outcomes as far as is possible.

8. Carry out appropriate and accurate recording of all contact with people that use the service.

9. Participate in all aspects of training and development as directed and to use all relevant learning opportunities to improve personal skills, effectiveness and efficiency of service delivery

10. Participate in supervision and reflective 1:2:1 and team meetings.

11. Maintain professional boundaries and adherence to GDPR.

12. Liaise with other agencies, volunteers and other staff members where necessary to access wider support.

**Knowledge, skills and experience**

1. A lived experience of managing your own long term health condition (whether that be Diabetes or another) and navigating services within Birmingham

2. Have a desire to put people at the heart of their own support and to work with them to identify and achieve their goals

3. Be a good communicator

4. Be proactive and positive

5. Be non
