Job Overview
We are seeking a dedicated and enthusiastic Community Assistant to join our team. As a Community Assistant, you will play a vital role in providing positive, person-centred care for our clients.

**Personal Care**:

- Provide assistance with activities of daily living, including personal hygiene, grooming, dressing, and mobility support as required.
- Administer medication according to prescribed schedules and guidelines, ensuring compliance with medication management protocols.
- Assist with meal preparation, feeding, and dietary management, adhering to specific nutritional requirements and preferences of clients

**Companionship and Social Engagement**:

- Engage clients in meaningful conversations and activities to promote community events and cognitive stimulation.
- Accompany clients on community outings, errands, and recreational activities to enhance their sense of independence and connection with the outside world.
- Foster a supportive and nurturing environment where clients feel valued, respected, and emotionally supported.

**Safety and Wellbeing**:

- Monitor clients for any signs of discomfort, distress, or changes in health status, promptly reporting concerns to the appropriate healthcare professionals or supervisors.
- Ensure a safe and hygienic environment for clients by maintaining cleanliness and organisation within their homes.
- Follow established infection control procedures and safety protocols to minimise the risk of accidents, injuries, or infections.

**Documentation and Reporting**:

- Maintain accurate and up-to-date records of client care activities, including daily progress notes, medication administration records, and incident reports.
- Communicate effectively with the care team, sharing relevant information and observations to ensure continuity of care and collaborative decision-making.

**Professional Development**:

- Participate in ongoing training and professional development opportunities to enhance your knowledge and skills in providing person-centred care to older adults.
- Adhere to the policies, procedures, and ethical standards outlined by Wildflower Home Care and regulatory authorities governing the provision of home care services.
