JOB SUMMARY / PURPOSE The Community Substance Misuse Practitioner will take responsibility for the key working or work alongside people who are key worked elsewhere within the team of a small clinical caseload of clients with substance misuse, supporting and including those with more complex health needs including co-occurring mental health / substance misuse and physical health complications. To support the Team Manager & Practice Lead in providing clinical leadership and supervision particularly in regard to complex health needs To work in conjunction with the Team Manager and take the lead on agreed areas in the overall development, monitoring and evaluation of the service. To work in close collaboration with other partner agencies and act as a source of specialist advice in regard to individual service user care and service delivery. To provide specialist guidance, consultations and support to the team with care planning and risk management.

MAIN DUTIES AND RESPONSIBILITIES Assessment & Care Planning & Risk assessment The post holder is required to act as key worker within the PCF framework for a reduced clinical caseload of service users with more complex needs, i.e. co-occurring mental health problems, substance misuse and physical health complications. This will include: Comprehensive assessment of the service users substance misuse problem including their health and wellbeing needs and level of motivation to change. agreeing a care plan /aftercare / discharge plan with the service user, which may include other professionals or onward referral as appropriate.

Implementation and review of the care plan and associated evaluation in line with the service. The post holder will be expected to carry out comprehensive assessments of risks to service users and others health and wellbeing. As part of their clinical duties the post holder will prepare risk assessment tools and management that are based on thorough risk assessment and will provide protective interventions to help manage risk. specific interventions / protective measures (child protection, domestic violence, protection of vulnerable adults etc) review of the risk assessment and crisis management plan at regular intervals and as circumstances change.

The post holder will have an understanding of the relevant legislative frameworks and will participate in relevant meetings related to the management of risk including Adult safeguarding, Child Protection meetings and Criminal Justice Risk Management Forums. Identify and support with a holistic care plan will, that identify goals and specific activities and interventions to be undertaken within the context of their overall care and their health and wellbeing needs. Enabling clients to address their emotional, mental, physical, social and spiritual health and wellbeing needs. The post holder is expected to respect peoples dignity, wishes and beliefs, involve them in shared decision-making and obtain their consent to share information with identified individuals / agencies on a need-to-know basis.

Interventions & treatment intervention are generally provided within a framework of PCF. This post holder will be responsible to deliver clinical supervision, consultations/specialist advice, case review, case management meeting, and complex client review. The post holder will care coordinate a reduced clinical caseload of service users likely with a dual diagnosis or those with significant substance misuse difficulties covering the citywide location. The post holder will plan, deliver and evaluate interventions and/or treatments when there are complex issues and/or serious mental illness The successful applicant will share the treatment plan of individuals routinely with the MDT Alert the line manager when there are unexpected changes in individuals health and wellbeing or level of risk, discuss and plan with the MDT and any other wider agencies Respond to, record and report any adverse event or incidents using the incident report systems within Livewell WORKING AS PART OF A TEAM The Community Substance Misuse Nurse will support the Team Manager & Practice lead in overseeing the effective delivery of the locality service and in providing clinical leadership and supervision for the Team, which may include leading clinical meetings or other tasks as directed by the Team Manager or Practice lead.

The post holder will hold line management responsibilities within the team, have a responsibility of completing line management supervision, annual appraisals and supporting staff in developing a personal development plan. The post holder will work in close collaboration with other partner agencies and act as a source of specialist advice in regard to client care and service delivery.
