Torridge Health Primary Care Network is a forward-thinking Primary Care Network situated in beautiful North Devon. We are looking to recruit Nurse Associates to support our Older Person Integrated Care and Home Visiting Teams.

Are you an NMC registered Nurse or Nurse Associate looking for a role working with a Primary Care Network as part of a multi-disciplinary team? We are a dynamic, proactive PCN team who are passionate about developing and delivering excellent quality local services to meet the needs of our patients.

The post holder will:
Work as part of the PCNs MDT to provide and monitor care, under direct or indirect supervision;
Improve safety and quality of care at every opportunity;
Contribute to the delivery of integrated care;
Provide support and supervision to training nursing associates, healthcare assistants, apprentices, and those on learning assignments/placements as required including delegation of tasks suitable to their role.

Support registered nurses to enable them to be able to focus on the more complex clinical care;
Develop relationships across the PCN to support integration of the role across health and social care including primary care, secondary care, and mental health;
Perform and record clinical observations such as blood pressure, temperature, respirations, oxygen saturations and pulse;
After undertaking additional training, administer and promote vaccinations and routine immunisations, perform ECGs, venepuncture, cervical smear tests, and other relevant clinical tasks as required by the PCN, and in line with your scope of practice;
Promote health and well-being to all patients, for example undertaking the NHS health checks;
Undertake assessments and reviews for individuals with dementia, mental health conditions, and learning disabilities;
Ability to makes judgements which require assessment of facts, e.g., escalating abnormal clinical observations, contraindications, emergency situations, wound care with a focus on pressure dressings etc. and deciding on implementation of care programmes where there are a number of options

Ability to plan and organise own workload for clinics, home visits and audits

Support the delivery of services to enhance care to care homes, the housebound and frailty population through virtual, telephone, face to face and home visit reviews.

Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services;
Care for individuals with dementia, mental health conditions and learning disabilities

Chaperone patients when required

Communicate proactively and effectively with all MDT colleagues across the PCN, attending and contributing to meetings as required;
Maintain accurate and contemporaneous patient health records;
Enhance own performance through continuous professional development, imparting own knowledge and behaviors to meet the needs of the service.

Support the coordination and delivery of MDTs within the PCN

Undertake all mandatory training and induction programs

Contribute to and embrace the spectrum of clinical governance

Contribute to public health campaigns (e.g., COVID-19 or flu clinics) through advice or direct care

Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner

Work with the general practice teams to ensure the practices are compliant with CQC standards.

**Quality**

Under supervision and support of GPs in practices, the post-holder will strive to maintain quality within the practices, and will:
Participate in clinical governance activity and contribute to the improvement in quality of health outcomes through audit, risk management and the Quality and Outcomes Framework.

Alert other team members to concerns about risk, quality and safety.

Participate in investigation of incidents and events as required.

Collect data for audit purposes and use clinical audit to monitor quality in the service and implement change according to findings.

Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the team’s performance.

Work effectively with individuals in other agencies to meet patient needs.
