**REPORTS TO**: The Practice Clinical Pharmacist

**HOURS**:37.5 hours per week

**Salary**:£11.42-£12 p/h

**Job Summary**

To manage and maintain patient’s medication accurately. Working closely with the in-house pharmacist. Processing medication requests. To implement medication changes that are authorised from GP’s/Pharmacist/ Hospitals.

**Responsibilities**:
The following are the core responsibilities of the medicine coordinator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

- Issue acute medication
- Manage medication tasks/queries from patients
- Add/Remove medication from patients as per internal tasks/letters from hospitals
- Process dosette box medication requests/alterations
- Process discharge summaries and consultant letters
- Input data into the patient’s healthcare records as necessary
- Answer incoming phone calls, transferring calls or dealing with the callers request appropriately
- Manage all administrative queries as necessary
- Maintain a clean, tidy, effective working area at all times
- Contact patients/pharmacies regarding medication changes
- Liaise with hospitals when required
- Book medication reviews with the pharmacist

In addition to the primary responsibilities, the medical administrator may be requested to:

- Partake in audit as directed by the audit lead
- Support reception staff, providing cover during staff absences
- Complete opening and closing procedures
- As required support reception in the management of repeat prescriptions, ensuring they are processed accurately and efficiently

**Service Delivery**

Staff at Eleanor Cross Healthcare must adhere to the information contained with practice policies and regional directives, ensuring protocols are always adhered to. Staff will be given detailed information during the induction process regarding policy and procedure.

**Security**

The security of the practice is the responsibility of all personnel. Staff must ensure they always remain vigilant and report any suspicious activity immediately to their line manager. Under no circumstances are staff to share the codes for the door locks to anyone and are to ensure that restricted areas remain effectively secured.
