**Am Y Gwasanaeth**
Mae cyfle cyffrous wedi codi o fewn y Gyfarwyddiaeth Tai a Chymunedau ar gyfer Swyddog Cyswllt Porth i Deuluoedd yng Ngwasanaeth Cyngor a Chymorth i Deuluoedd Caerdydd. Mae’r gwasanaeth hwn yn hanfodol i gyflawni dull ‘dim drws anghywir’ y Cyngor o sicrhau bod plant, pobl ifanc a’u teuluoedd yn cael y cymorth cywir ar yr adeg gywir. Drwy ddarparu cymorth ar y lefel isaf o ymyrraeth, nod y gwasanaeth newydd hwn yw lleihau nifer y teuluoedd sydd angen ymyrraeth adferol, gan sicrhau y gall pob teulu gyflawni eu canlyniadau gorau posibl.

**Am Y Swydd**
Bydd deiliad y swydd yn gweithio fel rhan o’r Tîm Porth i Deuluoedd (Pwynt Cyswllt Cyntaf e.e. ymholiadau ffôn, gwe ac e-bost) i ddarparu gwybodaeth, cyngor a chymorth fel rhan o Wasanaeth Cyngor a Chymorth i Deuluoedd Caerdydd.

Mae'r llinell ffôn ar agor o 8:30am - 5:00pm bob dydd ac mae patrwm shifft yn ei le ar gyfer cyflenwi. Mae patrymau sifft yn destun adolygiad ac anghenion busnes.

Byddwn yn cynnig:

**Beth Rydym Ei Eisiau Gennych**
Croesewir ceisiadau gan y rhai sydd â’r gallu i empathi, yn ogystal â sgiliau rhyngbersonol, gwrando a chyfathrebu rhagorol i feithrin cydberthynas effeithiol a rhoi cyngor cywir. Bydd deiliad y swydd yn gyfrifol am ymdrin yn brydlon ac yn briodol ag ymholiadau, gan nodi a, lle bo modd, diwallu anghenion pobl cyn gynted â phosibl. Bydd yr unigolyn cywir yn gallu cydnabod a oes unrhyw bryderon diogelu ac yn gallu ymgynghori a chysylltu â Gweithwyr Cymdeithasol a gweithwyr proffesiynol/asiantaethau partner eraill, yn ôl yr angen.

Mae'r rôl hefyd yn ei gwneud yn ofynnol i'r ymgeisydd llwyddiannus fod yn wydn wrth reoli galwadau niferus yn effeithiol a meddu ar y gallu i flaenoriaethu ac addasu i anghenion eu llwyth gwaith. Bydd gennych y sgiliau i aros yn ddigynnwrf o dan bwysau a byddwch yn gallu ysgogi eich hun mewn amgylchiadau heriol. Bydd angen i'r ymgeisydd fod yn gyfarwydd â defnyddio mapiau proses sy'n darparu atgyfeiriadau effeithlon i'w gwneud gyda'n partneriaid atgyfeirio.

Bydd gwybodaeth fanwl a chyfredol o wasanaethau sy'n cefnogi teuluoedd, y gallu i weithio ar draws systemau gweinyddol a gwybodaeth ymarferol gadarn o feddalwedd Microsoft yn ategu eich gallu i flaenoriaethu llwythi gwaith.
