**Location: Dorset (Royal Bournemouth Hospital and Poole Hospital)**

**Hours: 21 per week (service runs Mon - Fri, 9am - 5pm. Working alternate weekends and bank holidays)**

**Contract Type: Permanent**

**Salary: £19,838 pro rata, (£11,903 based on 21 hour working week) per annum**

**Driving: Full UK Manual Driving Licence to drive British Red Cross Vehicle**

Do you love helping people in need? What would you do if you had no family to support you when you were in need and in hospital? Or if you were in hospital and could go home but there was no one to take you?

That’s what this job is all about - helping patients being discharged from hospital have a safe journey home. It’s about ensuring their abode is liveable. Making sure they have everything they need to manage. Thus, reducing the risk of needing another hospital visit.

You will assist patients to be discharged in a safe and timely manner. Primarily from
**Royal Bournemouth Hospital**, but you will also support
**Poole Hospital** when needed.

**A day in the life of a Support Worker could involve**:

- Speaking with healthcare professionals about the support patients require
- Driving to different locations including hospital, doctor surgeries, pharmacies, shops and patient’s homes using BRC vehicles
- Collecting prescriptions, making sure there is food in the house, doing light household tasks
- Offering both emotional and practical support to people using our service
- Helping to assess our Service Users future needs and helping develop support plans
- Signposting to relevant agencies and community groups for support/engagement
- Ensuring our systems are updated so that we can track what support has been offered to patients

**To be a successful Support Worker, you’ll need**:

- A manual driving licence
- A caring, empathetic nature to be able to offer the best support be it on phone or face to face
- Basic IT knowledge, training will be provided on BRC systems
- An understanding of the services the NHS provide, to be able to offer the best advice and guidance to our service users

There is no personal care or administration of medication for this role.

**Wednesday 29th November 2023** with interviews to happen soon after.
