**Job overview**:
We have an exciting job opportunity working with a team of midwives and maternity support workers based at Royal Oldham Hospital.

We are looking for a motivated Community Clerk who enjoys working as part of a large team.

The position will require you to have excellent communication skills, be able to multi task and to have a passion for women, babies and their families.

**Main duties of the job**:
Ensure that women are signposted to the relevant departments and inform the maternity team as appropriate.

After appropriate training, take responsibility for accurate data entry and retrieval via computer e.g, P.A.S., Casenote tracking, pathology and other systems.

Ensure that discharged patient notes are returned promptly to appropriate secretaries or medical records department

Arrange follow-up appointments if required

Ensure that patient admission documentation is maintained appropriately and that a record is kept of clinical activity, as required

Liaise between the ward and other departments, including Pharmacy, Pathology and Stores

Ensure that patient’s notes are made available as required.

Liaise internally and externally with departments to arrange investigations and receive results and other information in a timely manner

Ensure that information is relayed appropriately to the multi-professional and/or multiagency teams to ensure the safe transfer and follow-up of patients
