Urology Renal Support Worker

The post holder will be an integral part of the Urology Cancer Hub Team, who works under the supervision of the Cancer & Performance Manager in the Urology Directorate. The main purpose of the role will be to provide dedicated patient centred coordination for patients who are selected for Renal Surveillance via Supported Self Management I. T. system.

The post holder will demonstrate excellent organisational skills, must be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for confidentiality and integrity. They would need excellent computer and data input skills.

Coordinates care by providing a single point of access for renal patients under going surveillance.

Has a sound understanding of the tumour specific pathways.

Develops good working relationships with local primary care teams.

Triage incoming calls and initiate appropriate response according to protocols and individual pathways, using good communication skills, basic clinical awareness and appropriate tools and procedures, liaising as appropriate when non-routine.

Refer all clinical and complex enquiries to the team for assessment and review.

Provide basic telephone advice and refer on or sign-post to other sources of support.

Coordinate the necessary assessments, appointments or investigations.

Add identified patient to Supported Self Management I. T. system.

Be able to Guide people through the use of this self management system.

Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit.

Maintain an accurate updated system on Supported Self Management I.T. system and track all investigations required.

Ensure that situations or enquiries requiring specialist intervention are referred appropriately and in a timely manner.

Coach patients and carers on how to make contact when they feel that their condition or needs have changed, including what to do out of hours.

Maintain an accurate updated system on Supported Self Management I.T. system and track all investigations required.

Deal with difficult and highly emotive situations in a sensitive manner using a high degree of empathy, understanding, diplomacy and integrity.

Ensure that situations or enquiries requiring specialist intervention are referred appropriately and in a timely manner. Coach patients and carers on how to make contact when they feel that their condition or needs have changed, including what to do out of hours. Demonstrate an awareness of the limits of own practice and knowledge and when to seek appropriate support/advice. Work across primary/secondary/tertiary care following the patient as the pathway dictates
