We are looking for a motivated individual with excellent communication, interpersonal and organisational skills who are dedicated to working as part of the team.

The post holder will act as an expert Metastatic Breast Cancer Clinical Nurse Specialist (CNS) providing advice and clinical intervention to this patient group. Promoting a seamless service within a multi-professional framework leading on service quality improvement, redesign and development. This will be achieved through the core dimensions of the role, clinical practice, leadership, and acting as the patient advocate.

Responsibilities will include the day-to-day management of the metastatic breast cancer patient case load, nurse led clinics, participation in ward based inpatient support, attendance at breast MDT, collation and execution of management plans. Advanced skills in clinical assessment, SACT, symptom control and an in-depth knowledge of treatment modalities.

We are looking for a Breast Cancer CNS'
- To act as a key worker for patients providing a link to metastatic breast cancer multi-disciplinary team providing care for patients diagnosed with metastatic breast cancer cancers referred to MSEFT.
- To provide specialist metastatic breast cancer nursing undertaking specialist assessment of patients nursing needs, plan, implement and evaluate care delivery using expert clinical knowledge and clinical skills.
- Co-ordinate and manage nurse-led clinics under the indirect supervision of the breast cancer/breast surgery Consultant team.
- Be responsible for own caseload of patients, ensuring this reflects the core components of the Clinical Nurse Specialist role, which involves high level of clinical decision making, advance clinical practice and providing consultations for healthcare professionals.
- Ensure good, accurate communication both verbally and via formal documentation of electronic sources with all members of the nursing, multidisciplinary team, patients and relatives.
- Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery.
- In partnership with lead clinicians, deliver health education and lifestyle advice, to empower patients and their carers to make informed choices about their health and wellbeing and increase concordance with suggested therapies and lifestyle changes.
