**Job overview**:
The main focus of the Pathway Facilitator role will be to track patients across their 18 week pathway, ensuring the patients and carer experience is central to every stage of the patient journey, and ensuring potential breaches of RTT (referral to treatment) waiting times targets are avoided or escalated to the relevant manager.

The post holder will proactively manage and track the progress of all referrals to ensure each stage of the pathway is fully planned and booked, whilst also working with colleagues throughout the Trust to ensure waiting time targets are achieved.

They will be required to assist in the data quality and validation, ensuring all patients have recorded pathway start dates, and support the management team to identify areas that require pathway improvements. Carry out any other tasks as required, commensurate with the grade of the post.

**Main duties of the job**:

- Track patients on 18-week pathways from referral to consultation, diagnostics, diagnosis and treatment ensuring there are no delays and the 18-week target is achieved.
- The facilitator will be required to communicate complex sensitive information/situations relating to patient pathways to a wide range of health professionals and use own initiative to seek professional advice as needed and autonomously act upon that information. This will include the ability to negotiate with a wide range of departments.
- To communicate regularly with managers and clinical teams to ensure that information requirements are met.
- Respond to any barriers identified in patients’ pathways and liaise effectively with medical and management colleagues regarding these.
- Maintain effective communication and act as a point of contact for colleagues within the division for 18-week pathways.
- Attend and participate in staff meetings giving accurate information of current 18-week performance position.
- Data Quality checks / reports / audits as required.
- Monitor and validate any patient lists as required.
