This is a very exciting opportunity for an ACP to join and integrate into the existing oncology team at Northampton General Hospital. You will be working as an autonomous practitioner, primarily working in the Chemotherapy Suite to carry out extended roles covering both SACT clinics and routine advanced physical assessments of the day care SACT patients.

The post holder will provide safe and effective care through expert knowledge, critical decision making and advanced skills. They will be responsible for independently managing care through a comprehensive assessment and physical examination of cancer patients who present via the planned and emergency route. The post holder will order and interpret diagnostic tests for patients presenting with oncological and haematological emergencies and will be responsible for compiling management plans, incorporating safe non-medical prescribing practice and will work as part of a wider multi-disciplinary team.

This is a new role, and the successful post holder will have completed an MSc in Advanced Clinical Practice and will also have skills in the early detection and intervention of clinical deterioration in existing or new medical conditions. You will be confident to identify and treat complications due to systemic anti-cancer therapy, radical/palliative radiotherapy, or hormonal or bisphosphonate therapy for metastatic disease. Some common examples of conditions you will see, assess and treat will include:

- Febrile neutropenia
- Thrombocytopenia associated with bleeding
- Dehydration
- Severe vomiting or diarrhoea
- Problems with central venous access device
- Needing symptom control
- Extravasation
- Drug reactions

Advanced clinical knowledge and skills are required to work as a core member of this team. You will possess evidence of clinical leadership and managing change and possess excellent communication skills with the ability to manage resources, people and situations effectively, whilst keeping a calm head in a challenging environment.

Clinical Practice

To autonomously assess, diagnose, plan and manage treatment interventions for patients presenting via the planned route

Hold telephone and face to face clinics

Undertake routine advanced physical assessment of the day care SACT patients, including assessing patients prior to each treatment using thorough history taking, clinical examination, arranging relevant investigations (e.g., blood tests, radiographs),

Prescribing SACT and other supportive medications

Arranging appropriate follow up when necessary

Presenting and attending at Consultant rounds

Preparing SACT clinic and Outpatient lists in conjunction with the team.

To provide support and advice to nursing, medical and allied health professional therapy staff in relation to the management of the SACT patient

To provide support and advice to patients and their carers relating to their chemotherapy and any procedures

To obtain informed consent from patients prior to starting their first chemotherapy cycle and ongoing cycles

To provide expert clinical advice and judgement on the management of acute oncology and haematology presentations

Act as an Independent Non-Medical Prescriber related to acute oncology and haematology presentations

Initiate relevant investigations to enable timely diagnosis and facilitate early decision making

Independently request and interpret diagnostic tests (e.g. blood tests/x-rays), providing appropriate and timely management as required

To utilise professional judgement and understand scope of practice. To be accountable for own actions and to seek help appropriately in practice

Use advanced communication skills to communicate sensitive, complex condition related information to patients/carers
- Provide an expert oncology and haematology opinion and review for patients referred to oncology and haematology, liaising with consultants to determine clinical manage
