Are you a Registered Nurse or Midwife with a special interest and commitment to improving the Health and Social outcomes for children and families? Harrogate District Foundation Trust is currently recruiting to full time Apprentice Health Visitor post (78 weeks). On completion of the course within timescales you will secure a permanent Health Visitor position.

Apprentices will be aligned to a practice assessor in either Durham/Darlington/Middlesbrough/Stockton.

The academic element of the course will be hosted by Teesside University.

As a Health Visitor you can make a real difference to children, families and communities. You will play a fundamental role in the delivery of the Healthy Child Programme, monitoring child development, safeguarding children, preventing social exclusion and deprivation and tackling important public health issues. Every day working as a Health Visitor is different, with responsibilities including home visits, running support groups for parents and working with a range of local children’s agencies and professionals

Health Visitors have the opportunity to work flexibly in the community and are a crucial part of the healthcare team. It is a diverse role which involves being part of a wider team who provide you with support as you work to improve the health of local families

Training

Teesside University, are undergoing course approval in line with the new NMC Specialist Community Public Health Nursing Standards of Proficiency (2022) and are looking to offer an apprenticeship route which leads to the Specialist Community Public Health Nursing qualification together with the academic award of a postgraduate diploma.

Applicants must have English and Maths at grade C or level 4 or Key Skill Level 2 in Maths and English.

Other duties of the role;
Making Every Contact Count to enable the opportunistic delivery of consistent and concise healthy lifestyle information.

The post holder will ensure inter-agency/professional partnership working between services, statutory and voluntary organisations to facilitate integrated approaches in meeting the needs of children

Working in an integrated way partnership with local communities to build community capacity; utilising strength-based approaches.

Hold a caseload of family’s children until the end of Early Years foundation stage (age 6 years), and beyond if working with identified families at universal plus and partnership plus at that stage, and be responsible for effective caseload/workload management, providing specialist evidence-based advice, information and support to the population served, team members and relevant agencies.

Lead and Deliver the Healthy Child Programme with a drive towards reducing the gap in health and social health inequalities.

Completion of comprehensive Health Needs assessments of clients, families and communities and facilitation of the mobilisation of appropriate resources to meet identified needs.

Using communication, listening and negotiation skills interpret family history, dynamics, lifestyle and the support available to a family in partnership with parents/carers to enable them to identify their own health needs and facilitate solutions to identified concerns and needs.

Promote the needs of children with SEND, young people with SEND and their families in the development of community child health services.

Promote the needs of children, young people and families from the GRT community in the development of community child health services

Deliver a place-based approach with Key Delivery Partners (KDPs) to support the needs of Children, Young People and Families alongside building community resilience. Supported by the Community Anchor, practitioners will work within the community to build capacity and increase resilience utilising an asset-based approach. Practitioners will need to work in collaboration with KDPs and the Voluntary and Community Sector to map and align services to meet population need, whilst developing innovative ways of wor
