To work as part of the Integrated Community Care team to prevent inappropriate hospital admission and have the capability to provide assessment and treatment of patients. To deliver a high standard of patient care using evidence based framework within East &North Hertfordshire. This post carries continuing responsibility for pro-active management of a caseload consisting of identified high intensity users with complex needs. The post holder will work autonomously to provide a comprehensive,advanced assessment and care package to the patients and families, utilizing support services and liaising with other agencies to ensure the complex needs of this client group are met.

To provide advanced nursing assessment, diagnostic, prescribing, care and treatment, case management and anticipatory care for patients with complex LTCs, marshalling resources, overseeing the quality of care from all agencies and co-coordinating acute, primary and social care ensuring that the focus of care remains at home and in the community where possible. A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010). Whilst it is not a legal requirement, Hertfordshire Community NHS Trust encourages and supports all staff to be vaccinated as this remains the best line of defence against COVID-19.
