We aim to provide a high standard of patient care using an evidence-based framework.
Successful applicants will be responsible for assessment, planning, implementation, and evaluation of individualised nursing care to patients living in the community. You will manage a caseload of a smaller area within the locality, working with your team to oversee high quality care for all our patients.
