We have an exciting opportunity for a Band 4 Senior Support Worker (SSW) to join us and support delivery of care to adults with a learning disability and additional complex health needs.
The SSW will support the clinicians - Occupational Therapists, Physiotherapists, Speech and Language Therapists, Dietitians, Community Nurses and Clinical Psychologists to deliver a wide range of therapeutic interventions.
The SSW will deliver physiotherapy programmes with Band 3 Therapy Assistants such as hydrotherapy and rebound therapy in Ventures Therapy Department and in community-based facilities.
The SSW will support Occupational Therapists to deliver a range of treatment interventions including supporting service users with anxiety to access community resources.
They will also support Community Nurses and Psychologists in behaviour assessments and gathering information for formulation of behavioural plans in line with the Positive Behaviour Support Framework.
The SSW will have regular clinical and operational supervision from clinicians in the team. There will be opportunities for training and development to meet the demands of this exciting and varied role. We will offer you the opportunity to develop your skills through supervision and continuing practice development.
An ability to travel throughout the Leeds area to meet the demands of the post is essential therefore the applicant must be able to drive and have access to a vehicle.
The Leeds and York Partnership NHS Foundation Trust (LYPFT) is the main provider of Mental Health and Learning Disability services in Leeds. We also provide specialist inpatient services in York and some highly specialised services across the country. The role of the Senior Support worker is to work with the multi professional team and the registered practitioner(s) to assess, plan, organise and deliver clinical programmes as agreed with supervising clinician(s). This work will be in accordance with clinical governance processes and professional standards. The post holder will utilise their skills and knowledge to undertake assessments and carry out treatment both in Ventures Therapy Service and community areas as appropriate. The post holder will also provide educative and peer support to therapy assistants / associate practitioners. The post holder will work collaboratively with the service user, and all involved in their care, ensuring that issues of consent are fully addressed and that all approaches and plans are created in co-production with the service user, their family and support network.
To provide effective evidence-based care for service users referred to the service under the supervision of qualified clinicians in order to perform non-complex activities.
To carry out assessments for service users who have been delegated by the registered practitioner and develop individual plans of care to meet the immediate and on-going needs of these service user and their carers.
To take into account the specific physical, emotional, psychological, social, cultural and spiritual needs of service users when developing care plans.
To participate in and contribute to the delivery of complex care as prescribed by the registered practitioner.
Work with the multi-disciplinary team and other providers and referral systems to ensure that service user and carer needs are met, within the scope of practice.
To work closely with all members of the multi-disciplinary team, seeking opportunities to promote physical and mental health and contribute to maintaining service users care in the community.
