An exciting opportunity has arisen to join our Health Team. We are looking for the right person to deliver our social prescribing service based in the Sheffield University Health Service, supporting students of all ages.

The postholder will be responsible for all aspects of day to day delivery of the service including; managing referrals, one-to-one work with patients to link them to agencies and activities that will improve their health & wellbeing, promoting the service across the practices, communicating with clinicians, researching beneficial services, and keeping accurate and confidential records.

You **must** have experience of working with individuals in a non-judgemental and encouraging manner, an ability to support individuals in defining meaningful goals that will improve their health & wellbeing, good organisational skills and the ability to build positive working relationships with partner organisations.

**Purpose of Job**

The post-holder will act as a single point of access for patients referred through our social prescribing service, based in the University of Sheffield’s Health Service. They will support patients to access services that can enable them to improve their own health and achieve their personal goals. The post-holder will work with partners to promote the social prescribing service in the local community and relevant professionals.

**Responsibilities**
- To work flexibly as a member of the Zest for Health team, liaising with colleagues as appropriate.
- To work with University Health Centre and other organisations to identify and engage patients who would benefit from community-based support, especially those at risk of declining health and wellbeing and carers.
- Take an active part in promoting the social prescribing service to the participating GP surgeries, attending practice staff/Multi-Disciplinary Team meetings to promote the service and to discuss patient care
- Support the promotion of health and health improvement within the local community as appropriate, signposting where appropriate and maintaining good relationships with local agencies.
- Take an active part in developing own knowledge and skills.
- To carry out any associated administrative duties as required.
- To ensure the service works towards best practice in equality of opportunity.
- To undertake training and other development activity in line with organisational and personal development needs.
- To undertake any other duties as agreed with the Line Manager.
- To be fully responsible for all referrals through the social prescribing service. You will:

- Act as a single point of contact for referrals - ensuring referral guidelines and referral forms are easily accessible and used by GPs.
- Meet with and triage all clients referred to the social prescribing service, work with them to assess their needs and support them to access the right services or activities that can best help them improve their health and wellbeing.
- Help people identify the wider issues that impact on their health and wellbeing, such as debt, poor housing, being unemployed, loneliness and caring responsibilities
- Provide accurate information in line with government guidelines about health issues including diabetes, heart health, physical activity, smoking and healthy eating.
- Support individuals in an encouraging and non-judgemental manner to identify goals that will help them make healthy lifestyle changes and assist them in finding appropriate services/activities
- Support clients to understand how their behaviour/s might affect their health and wellbeing and understand the barriers they may have in relation to changing this.
- Keep up-to-date regarding services that could be of benefit to clients.
- Signpost clients to access other agencies and services for information, support and resources (including internally at both the University and at Zest)
- Respond in a timely manner to client referrals, in line with the contract, reporting back to referring GPs.
- Keep accurate records and enter client data onto the bespoke monitoring database.
- Publicise the social prescribing service within the local student population.
- Report to Health Services Coordinator on the progress of the case load on a regular basis and discuss any arising issues.
