Cancer Pathway Navigator

An Exciting opportunity has arisen for a Cancer Pathway Navigator within the Gynae-oncology Service. The aim of the Patient Navigator role is to help support and navigate patients, their families and carers through a haematology cancer pathway. The service's prime aim is to be patient-focused and proactive in seeing that the patient receives the very best cancer pathway experience with continual communication and support in a timely manner.
You will be involved throughout the patient journey from referral to follow up within the hospital setting, working directly and indirectly with patients and their carers, assisting the clinical team in coordinating the patient pathway.
The role requires someone with exceptional organisational skills who can demonstrate great communication and interpersonal skills. A caring and sensitive attitude is essential as you will be dealing with patients, their families and carers. You will have excellent listening skills and be able to deal with people both internally and externally in a sensitive and professional manner and be confident using patient administration systems and database packages.
The post holder will be expected to work independently within the appropriate policies and procedures for the service and will actively support and contribute to the requirements of the service and ensure compliance with performance targets and quality standards required at a local and national level.
The post will focus primarily on working collaboratively with the clinical team supporting the diagnostic pathway to ensure patients are supported through their cancer diagnostic pathway in a seamless and safe manner.
Provide explanations of procedure and processes to patients and their family regarding the pathway, verbally and by sending out relevant trust leaflets and instruction packs where appropriate.

Communication
To act as centralised point of contact for patients, receiving, triaging and responding to face to face, telephone and written queries, liaising and escalating to clinical team/ operational teams as required.
Demonstrate high levels of empathy, sensitivity, calm and professionalism in all communications with patients and their families, recognising where referral to other sources of advice and support may be beneficial e.g., nursing or clinical colleagues.
Be aware of the potential for emotional, psychological and physical distress and symptoms that the patient may be experiencing, liaise and escalate to the specialist nurses or clinicians as appropriate.
Document and monitor all aspects of the care coordination and service delivery.
Ensure timely communication is maintained with the GP and other care providers as required, including a discharge summary to be sent to the GP.
Assist people to access appropriate information and support by sign posting to a range of support services and encourage self-management where appropriate.
Deal with concerns/complaints as appropriate, promptly escalating where necessary to the appropriate manager.
Demonstrate advanced keyboard and IT literacy skills
