A great opportunity has arisen to appoint a part time Lead Cancer support worker at University Hospitals Dorset.

The delivery of the personalised cancer care and support agenda at UHD, is an important component of the cancer patient's pathway. Providing reliable, seamless support, information and communication is a vital part of cancer care; and one that can improve both the quality of care, and the experience of care.

Reporting to the Lead Cancer Nurse, the post holder will be responsible for operational management and leadership of the Cancer Support Worker (CSW) team. They will support the Macmillan Personalised Cancer care project lead and will work closely with clinical teams to play a key role in the implementation of the wider delivery of Personalised Care and Support (PCS) agenda for all cancer patients with the trust.

The Lead Cancer Support Worker will provide guidance and leadership, enabling the support worker teams to be an integral part of the multidisciplinary team in caring for the physical, social and psychological needs of patients diagnosed with cancer and their carers throughout the care treatment pathway and after care support.

The post-holder will also manage specific personalised cancer care projects such as a Cancer volunteer pilot, in collaboration with the trusts Volunteer team. Experience working with volunteers would be beneficial, as would experience within cancer care.

The post holder will work independently within the remit of the role but under the guidance and supervision of the Lead Cancer Nurse.

To support the implementation and provision of Personalised Care and support at UHD.

Link with local and national teams to keep up to date on CSW development and recommendations.

To proactively and efficiently lead, manage and motivate a first-class customer focused cancer support worker team.

In conjunction with the Clinical Nurse Specialists, Lead cancer nurse and RMS project manager, support the CSW teams to manage patients on patient led risk stratified follow up.

To support clinical teams in the planning and coordination of generic and site-specific wellbeing workshops and collate patient evaluation.

To actively encourage patient engagement in the on-going development of personalised care and support initiatives.

To have awareness of UHD’s Personalised Care and Support (PCS) objectives and relevant workstreams

As Lead cancer support worker, you will support teams to provide regular supported conversations with the person living with cancer, assessing their holistic needs, and supporting them to self-manage by providing relevant information and advice, and signposting to other resources and services available to meet their identified needs

To guide and educate teams to follow clear pathways to achieve effective holistic needs assessment.

To guide and educate teams to further support and manage patients at all stages of their cancer pathway; ultimately to optimise patients access to the right help at the right time, providing a seamless experience of care.

To support the implementation of new cancer care pathways such as stratified follow up/RMS.

Line management responsibility of the Cancer Support worker team including the supervision, recruitment, appraisal, performance and the utilisation, delivery and adherence to trust policies and procedures. This will be in collaboration with the CNS’s that work directly with the site specialist cancer support workers.

To encourage the development of any staff directly managed to their full potential through systematic individual performance development review and the establishment of personal development plans, reflecting the needs of the organisation and the individual.
