**Are you passionate about providing high quality patient care? The Launceston and Tamar Valley Community Nursing team would like you to support them in delivering high quality care and support to people approaching the end of their lives.**

**Main duties of the job**:
This is an exciting and fulfilling role and you will need to be able to demonstrate a real enthusiasm for nursing people in the community especially those approaching the end of their lives as well as a desire for on-going professional development.

You should also have an understanding of the changing climate of care delivery in the community with the ability to travel independently across the area to fulfil the requirements being essential.

We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health. We also provide specialist support to people with dementia or a learning disability.

Over 4,000 people make up the Trust. This includes doctors, nurses, therapists, plus admin and support staff.

Just over 532,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon.
