Band 7 full time Practice Educator - Community

In Croydon, Health and Social care providers and commissioners have formed the Croydon Alliance Board with the aim of providing a whole system transformation that will deliver the outcomes our over 65’s have specified that they want.
To act as a role model and demonstrate expertise in delivering evidence based practice. To support Matrons in mentoring clinical
practice and meeting professional development needs of all staff to ensure that patient care standards and their experience are exemplary and any poor practice is identified and action plans implemented. Support mentors with completion of mentors triennial review.
To work in accordance with the Department of Health, Nursing and Midwifery Council and trust policies relating to infection control, safeguarding children/adults, standards of record
keeping, medicines management, learning disabilities, privacy and dignity and other policies as appropriate to the designated areas.
Be responsive to the changing needs within the Directorate in relation to local and national policy and guidance.
To provide leadership and support to promote and maintain high standards of patient care
Croydon Health Services NHS Trust provide hospital and community services from a number of community and specialist clinics throughout Croydon. The Trust was formed on 1st August 2010 through the integration of Croydon Community Health Services and Mayday Healthcare NHS Trust. Around 4,100 staff provide services for a population of over 360,000 people who are relatively young with a high level of ethnic diversity.
Our main hospital site, Croydon University Hospital is one of the busiest in London. Our experienced district nursing teams, Allied Health Professionals and community matrons look after people of all ages across Croydon and our Children’s Hospital at Home cares for children with long-term conditions without them having to come to hospital.

To act as a role model and demonstrate expertise in delivering evidence based practice
To support cluster matrons/ward managers/sisters/charge nurses/ district nurses, caseload holder in mentoring clinical practice and meeting professional development needs of the nursing team to ensure that patients care standard and their experience are exemplary and any poor practice is identified and actions implemented
To actively participate in campaigns related to improving the patients experience, for example Saving Lives, Score Cards and Productive Ward processes Outcome based commissioning
To work in accordance with the Department of Health, Nursing and Midwifery Council and trust policies relating to infection control, safeguarding children/adults, standards of record keeping, medicines management, learning disabilities, privacy and dignity and other policies as appropriate to the specialist area
Contribute to the creation of an environment in which change can be initiated and supported.
Be responsive to the changing needs within the Directorate in relation to local and national policy and guidance.
To provide leadership and support to promote and maintain high standards of patient care
To work clinically in practice areas 50% of the tie in an educational capacity and provide clinical education in practice.
